The situation

Digital QA was in place. Errors still shipped.

Staci Americas is a national 3PL processing more than 25,000 orders per day across 19 pack stations. They had already invested in digital quality assurance, scan verification was running at every station. Every barcode was scanned. Every order was "confirmed."

But scans confirm that a barcode was read. They don't confirm what physically went in the box.

Items that looked similar but weren't the right SKU. Inserts that were required by the client SOP but forgotten. Packaging that didn't match the brand's spec. Scan-verify said "packed." The customer said otherwise.
